We adapted a US Centers for Disease Control and Prevention (CDC) resource into Japanese for people preparing to see a healthcare provider about post-COVID symptoms. Preparing in advance may help you explain what you want to discuss, even when the visit is short.

Download the Japanese handout as a PDF or Word file.

Before the appointment

If this is your first visit to a provider in particular, it can help to list the healthcare providers you currently see or have seen in the past and your current and past medical history.

Write a brief account of your COVID-19 illness and symptoms since then. You might include the symptoms that you think began after the infection and the following details:

  • The date you first noticed COVID-19 symptoms and when symptoms appeared.
  • The date of a positive COVID-19 test, if you know it.
  • Past treatments or tests related to your post-COVID symptoms, such as blood tests and X-rays.
  • What makes the symptoms worse and how often they occur.
  • How the symptoms affect activities such as everyday tasks, work or school, and how you feel now.
  • Examples of your best and worst days since COVID-19.
  • The symptom or problem you most want to discuss with your provider.

Bring a list of the medicines and supplements you take. Many providers find this information helpful. Bring your medication record booklet if you have one.

Consider asking a trusted friend or family member to talk with you just before and after the visit. They may help you fill in the form and recall what you discussed while it is still fresh in your mind. If the provider permits it, you might also ask them to attend the appointment with you.
